Fill out the registration form in its entirety. (Download form here)  Please include e-mail information as this is a wonderful way for us to communicate with our families, both individually and as a school-wide tool.  We will not sell or use your email in any form of abuse.

Your teacher will be notified that registration and payment have been received.  To insure priority scheduling, turn your registration in early!

SUMMER REGISTRATION BROCHURE

CMMS policy states you must register with full tuition paid by one of the methods listed below before your first lesson can be scheduled.

By returning the registration form, you are indicating acceptance of all CMMS policies.

Payment Options:

Bi-Semester payment option:

The first b i-semester payment is due by August 20th .  This payment should include $22.50 for the first semester administrative fee (see below) plus the $25 annual family registration fee.

You will receive an invoice at the end of September for your next payment which is due by October 15th and will include a $10 split payment fee.

There is a 10-day grace period, during which time no late fees will be assessed.  If payment is not received within 10 days of the due date, late fees will be assessed in accordance with CMMS policies.  Any payment not received within 20 days of the due date will require the teacher to discontinue lessons until further payment is received.

Semester payment option:

Your first payment is due by August 20th (or at the time of registration, if you start mid-term).  This payment should include $22.50 for the first semester administrative fee (see below) plus the $25 annual family registration fee.

You will receive an invoice in late November for your next payment, due December 15th.  

Yearly payment option:

Payment for the entire year is  due by August 20th (or at the time of registration, if you start mid-term).  This payment should include the $25 annual family registration fee.  No administrative fee is assessed on families who pay for the full year in advance by the due date.
