Please download, print and fill out the registration form in its entirety.   Please also include e-mail information as this is a wonderful way for us to communicate with our families, both individually and as a school-wide tool.  We do not sell or otherwise distribute e-mail addresses.

Your teacher will be notified that registration and payment have been received.  To insure priority scheduling, turn your registration in early!

CMMS policy states you must register with full tuition paid by one of the methods listed below before your first lesson can be scheduled.

By returning the registration form, you are indicating acceptance of all CMMS policies.

 

Payment Options Overview

Bi-Semester payment option:

The first bi-semester payment is due by August 24th .  This payment should include the $25 annual family registration fee.

You will receive an invoice at the end of September for your next payment which is due by October 15th and will include a $10 split payment fee. Subsequent billings will be on November 30th (due December 20th) and February 15th (due March 1st). There is a 10-day grace period, during which time no late fees will be assessed.  If payment is not received during the grace period, late fees will be assessed in accordance with CMMS Policies.  Any payment not received within 20 days of the due date will require the teacher to discontinue lessons until further payment is received.

Semester payment option:

Your first payment is due by August 24th (or at the time of registration, if you start mid-term).  This payment should include the $25 annual family registration fee. You will receive an invoice on November 30th for the second semester, due on December 20th.

Yearly payment option:

Payment for the entire year is  due by August 24th (or at the time of registration, if you start mid-term).  CMMS waives the annual family registration fee for families choosing this option.
